Privacy Policy


147 Elder Street is a health information custodian within the meaning of the Personal Health Information Protection Act, 2004. As such, the we must comply with certain requirements regarding the collection, use and disclosure of a Resident’s personal health information, and to protect such information from unauthorized collections, uses and disclosures. The Residence will only collect, use and disclose a Resident’s personal health information with the consent of a Resident or the Resident’s substitute decision-maker, subject to limited exceptions.

Residents will receive information about the Residence’s privacy policy and information practices with their Information Package. Also, at the time that a Resident commences his or her residency, he or she will be asked to sign a consent form with respect to the collection, use and disclosure of their personal health information by the Residence.

Please note that sections 77 and 80 of the Retirement Homes Act, 2010 authorize an inspector or an investigator respectively to inspect, copy and remove records containing a Resident’s personal information, including personal health information, from the Residence for the purpose of determining whether the Licensee is in compliance with the requirements of the Retirement Homes Act, 2010.
